The whole idea of picking one insurance plan that is the right fit for 310 million people seems ludicrous to me.

How about insurance companies get to decide what kinds of plans to offer and everybody uses their own money to pick the plan that fits them best?  Just like buying a shirt.  And for those who cannot afford a plan, they go on Medicaid.
